We're proud to announce the Iowa Pharmacy Association has awarded the GHG 3rd Street clinic the Patient Care Partner award for their efforts in making the Chronic Care Management program a success. This program allows our providers to refer high risk patients to a team of pharmacists for chronic care management and has been a successful partnership to capture the patients who might fall through the gaps of the system, whether it be with education, medication compliance, medication affordability, and keeping these high utilizers out of the emergency department. They will receive the award at an in-person event in Des Moines in September. Congrats to the team on a well deserved award! #InThisTogether
